End of life care

End of life care helps those with advanced, progressive, incurable illness to live as well as possible until they die.

NHS Greater Preston CCG wants to ensure that all residents have access to high quality end of life care, and we want to make sure that our population are well informed and able to access co-ordinated care that identifies individual's preferences and wishes.

The CCG's five year strategic plan outlines end of life care as a priority, so as a result, the CCG's end of life strategy has been developed with extensive input from stakeholders within the integrated health and social care economy including Lancashire Teaching Hospitals NHS Foundation Trust, Lancashire Care NHS Foundation Trust, Lancashire County Council, St Catherine’s Hospice Care and North West Ambulance Service NHS Trust.

The CCG's Patient Advisory Group have also provided input to the development of the strategy.
